Goals and Objectives: specific goals and their desired outcomes

Other Funding: other current or projected sources of revenue
Information: mission statement, board of directors, history and track record community/population served
Agency and Project Budgets: statement of project costs, how much needed, how funds will be spent—and the applicant organization’s annual budget
